Abiraterone Acetate was approved for use in the United States in April of 2011. The results of a large clinical trial demonstrated Abiraterone increased survival approximately by four months longer then for men who received placebo for men with advanced prostate cancer. This is a exciting new treatment fo rmen with prostate cancer.

How was abiraterone discovered?
For many years it was known that giving a medication called ketoconazole, used to treat nail fungus, had a secondary effect of lowering men’s PSA. Ketoconazole lowered the PSA by blocking male hormones produced in the body from the adrenal glands. Therefore ketoconazole was used for many years as an “off label” use to treat advanced prostate cancer. Unfortunately ketoconazole has many bad side effects and cannot be used with multiple other medications. Thus abiraterone was developed to be very similar to ketoconazole, both in molecular shape and function, but it does not have nearly as many side effects.

What is abiraterone?
Abiraterone is a new medication, given as a pill once a day, for the treatment of advanced prostate cancer. The primary treatment of advanced prostate cancer is to lower your testosterone, either by surgical removal of your testicles (orchiectomy) or by receiving a hormone injection. This lowering of your testosterone does control the prostate cancer, usually for several years, until the cancer begins to grow again. Abiraterone is a medication that blocks other male hormones in the body, primarily produced in the adrenal glands, that works in conjunction with your hormone injection. Thus abiraterone is given when lowering your testosterone by hormone injections has stopped controlling your cancer.

Can I take my hormone shots intermittently?
Yes. In a recent study it was found that men who received their hormone shots intermittently lived just as long as those who received the shots continuously. The benefit of having the shots intermittently meant less hot flashes, fatigue, and other side effects from the hormone shots. Talk to you doctor for further information.

Will docetaxel chemotherapy help me?
Sometimes. You have approximately a 50% chance that docetaxel will help control your cancer. This would include lowering your PSA, and possibly improving your CT scans or bone scans. The control of the cancer can range from only a few months to a few years while on chemotherapy.
